A meeting of the full Long Island Health Collaborative members. We discuss our programs and initiatives, with member updates on our work in assisting the full spectrum of health and social service providers. We collaborate on bettering our culture of health, the social determinants of health, and the healthcare system, especially in the areas of chronic disease and mental health, providing more efficient and cost-effective care for all Long Islanders through population health activities.
