The Diabetes Wellness Center at Long Island Community Hospital offers a Diabetes Self Management Education Program that enables participants to make proactive, permanent and positive lifestyle changes tailored to their specific needs. Program participants partner with Diabetes Educators to learn how to manage their condition so that they can live a healthier life with diabetes.
